If you plan on participating in this event, there will also be an option to provide a one-time gift at the end of your registration form or you can do so by filling out the info below:

If you would like to become a regular donor, please call (765) 742-1567, Ext 3058 to set up your regular gift. If you are participating in the 5K and become a regular donor, you will also receive a $5 promo code off of the cost of registration.
